Once you have been booked in please bring with you the following items:
1) Photo I.D. e.g. Full Passport or photo Drivers Licence
2) Please remember to bring your GP medical summary and list of prescribed medications.
3) Your GP's details
4)Bring along form D4 Medical Examination Report Questionnaire from the DVLA (please do not complete the form, just bring it along). Click here to download a copy.
5) Bring your optician’s prescription, if you use glasses for driving
